DePaul's Vaccine FAQs
COVID-19 vaccination and booster requirements
In the spirit of caring for each other and for our surrounding community, DePaul is requiring vaccinations for COVID-19, including the booster when applicable. The due date for the booster depends on the vaccination received:
- Pfizer — booster five months after initial doses
- Moderna — booster five months after initial doses
- Johnson & Johnson — booster two months after initial doses
- If you were vaccinated outside of the United States, please visit the Centers for Disease Control and Prevention's website for details on booster dose eligibility.
This requirement applies to students, faculty and staff.
COVID-19 vaccines, including boosters, are widely available throughout the city, including at doctors’ offices, hospitals, pharmacies, community health centers, city-operated clinics, pop-up locations, and other special events. All COVID-19 vaccines are offered at no cost, regardless of ability to pay or immigration status. No government ID or insurance is required.

Students
Faculty and staff
- Visit
https://hitl-depaul.secure.force.com/DePaulCOVIDTracking.
- As instructed, enter your 7- digit DePaul ID number. It can be found on your DePaul ID or in Blue Sky as your "person number."

Vaccinerecords@mailing.depaul.edu will send an email to your DePaul email address with a secure link.
- Upon receiving the email, click on the link to securely submit your vaccine information and to upload a photo of your vaccination card.

Please complete all fields, including the Vaccine Booster Dose Date and the Booster Dose Vaccination Lot #.
Employee vaccination records will be maintained separately from employee personnel files, will be stored securely in accordance with the requirements of DePaul’s
Access to and Responsible Use of Data policy, and will only be shared only to the extent necessary to administer the program and ensure compliance with the university’s program.
Reminders

If you are eligible, you may request a medical or religious exemption: Email
employeerelations@depaul.edu or visit the
Human Resources website. Note: Anyone who was previous approved for a medical or religious exemption for the COVID-19 vaccine requirement
does not need to request an additional accommodation for the booster.

If you were recently diagnosed with COVID-19: You may not be eligible for a booster. Please have your physician provide a written statement validating both the diagnosis and the waiting period, which you can then use to submit a request for a medical exemption. As soon as the waiting period expires, you will be required to submit documentation for your booster.

Paid time off: Employees may use paid time off to get a COVID-19 vaccine or to recover from the vaccine's side effects. For more information,

How to schedule a vaccine appointment
There are multiple ways to receive a COVID-19 vaccine; some require appointments and some don't. Please check the information associated with the vaccination site closely to see if an appointment is required.
